Steps for Veterans Requesting to Be Seen by a CCN Provider
- Contact Your Primary Care Provider
The preferred and most efficient way is through the VA’s healthcare online messaging system. Let your provider know you would like them to submit a referral for an Audiology appointment. - Confirm the Referral
Once your provider confirms the referral has been submitted, please allow 2–3 business days for the Community Care Network (CCN) to receive it.- You may contact the CCN at 210-949-3850 (select option 1) to verify receipt of your referral.

When speaking with the CCN, request to schedule your audiology appointment specifically with:

After the CCN confirms your referral has been sent to us, please call Precision Hearing Conservation at 210-695-4708 to let us know.
